Authorization Form

CREMATION IS IRREVERSIBLE AND FINAL.  PLEASE READ THIS DOCUMENT CAREFULLY BEFORE SIGNING.

IDENTIFICATION:  I Pet Owner, hereby certify that I am the legal representative of (my pet), and have full authority to authorize the cremation, processing, and disposition of the cremated remains of my pet. I request and authorize, ALL PETS CREMATION & FUNERAL CENTER, to take possession of, cremate, and process the remains of my pet.

HOLD HARMLESS STATEMENT: I Pet Owner agree to hold harmless the company, its affiliates, agents, employees or assigns, of and from any and all claims, demands, causes, causes of action, and suits of every kind, nature and description in law or equity, including any legal fees, costs and expenses of litigation, arising as a result of, based upon or connected with this authorization, claims brought about by any other person(s) claiming the right to control the disposition of said pet, or any other action performed by the Company, its affiliates, agents, employees, or assigns, pursuant to this authorization, excepting only acts of willful negligence.

FINAL DISPOSITION: Once the cremation has taken place, I the Pet Owner, hereby authorize the Company to ship, release or otherwise dispose of the cremated remains as specified.
